• Please include the links/email addresses of your 3 most recent provider references along with your method of contact when contacting them (preferably your phone number). Please do not send another provider’s phone number; I will not contact her via text.

If you’re P411 verified feel free to send a client appointment request. Otherwise please hold any questions about appointments until after you have been verified.
